    For family-centered events and invitations hosted by Lansdowne Children's Centre.

  • This event form is used exclusively for activities where Lansdowne Children's Centre does not provide childminding at the event. Children of all ages must attend with and be supervised by a guardian/caregiver.

    Eligible attendees: to be eligible to attend this event as a guest of Lansdowne you must have a child registered with SmartStart Hub who is in active service with Lansdowne Children’s Centre or on a current Lansdowne wait list for assessment or services.

  • IMPORTANT: Adult attendees will be asked to please provide Photo Identification during check-in registration to verify ID prior to entering the event. A valid driver's license, health card, passport, or student card is acceptable. Failure to provide valid ID can result in denial of access to the event.

    There may be media recording photo/audio/video at this event. These materials are part of Lansdowne Children's Centre and community storytelling, shared on a variety of media platforms for the purpose of promoting Lansdowne community engagement, impact and service awareness, and philanthropic interest.

    You may be asked to sign an event release form at check in registration. It is your right to decline permission to use any recording of you or any member(s) of your group. You may be asked to wear an identification band or sticker to help event organizers respect your wishes when reviewing recordings for potential publication. Those persons identified in review will be removed or otherwise hidden from any publication.

    I understand that participation in a Lansdowne Children's Centre event may involve attending indoor and/or outdoor activities on grounds not under the control or management of Lansdowne Children's Centre.

    I acknowledge that attending any event property may involve risks outside the control of Lansdowne Children's Centre including uneven ground, flora, fauna, weather conditions, slips, trips, falls, and other potential hazards associated with event environments such as equipment, decorations, lights and sounds.

    I agree to supervise and be responsible for any child(ren) attending with me and included on this registration form. I agree to follow instructions provided by Lansdowne Children's Centre staff and event personnel.

    By participating in this event, I voluntarily assume these risks for myself and any child(ren) under my care and guardianship and release Lansdowne Children's Centre and its employees, volunteers, and representatives, and the event host  venue, and its owners, employees, and representatives, as well as any event sponsors or partners, from any claims arising from injury, illness, loss, or damage related to participation in this event, except where liability cannot legally be excluded.

    I understand that I and my party may be asked to sign a waiver form at check in registration prior to entering this event, and that agreeing to these terms is a requirement to be permitted access to the grounds and available activities.
